Managed Care Insurance Carriers
If you are covered by a managed care company, we will collect co-pays and deductibles at the time of service and wait 30 days for payment by the company. If, after this time, APCC has not received payment, we will require you to follow up with the insurance company to make sure additional information is not needed from you.
Federal and State Government Programs (Champus, Medicare, Medicaid)
If you are covered under one of the above governmental programs, we will collect co-pays and deductibles at the time of service. Whenever possible, APCC will submit claims to your secondary insurance carrier as well.
Medicaid allows 12 visits and $500.00 for lab and x-ray services per fiscal year (July 1 thru June 30) for ages 20 and above. If you have exceeded this amount, you will be required to pay at the time of service. We will submit an extension of benefit request and will gladly refund you the amount paid after the approval of the extension and Medicaid has received payment. Remember, one of our staff physicians must be selected as your primary care physician. If you are unsure of who your primary care physician is, you can contact connect care at 1-800-275-1131.
*Please remember to bring your insurance card with you.
